BLKMYNDZ is committed to connecting Black communities with trusted, culturally aligned mental health, wellness, and healthcare professionals. To protect the integrity of our directory and the people who rely on it, every provider application undergoes a manual review and credential verification process before acceptance.
This Provider Agreement outlines the terms and conditions governing participation in the BLKMYNDZ Provider Network. By applying for or maintaining a provider profile, you agree to comply with the requirements described in this Agreement, as well as our Terms of Service, Privacy Policy, Verification & Credentialing Policy, and Community Guidelines.
Participation in the BLKMYNDZ Provider Network is a privilege, not a right. Acceptance into the directory is determined at the sole discretion of BLKMYNDZ following successful credential verification and review of the information submitted during the application process.
1. Eligibility
To be eligible for inclusion in the BLKMYNDZ Provider Network, applicants must:
• Be at least 18 years of age.
• Hold all licenses, certifications, or credentials required to legally practice within their profession and jurisdiction, where applicable.
• Provide complete, accurate, and truthful information throughout the application process.
• Consent to BLKMYNDZ’s credential verification procedures, including professional license verification and any additional screening described in the Verification & Credentialing Policy.
• Maintain professional conduct that aligns with the mission and standards of BLKMYNDZ.
Submission of an application does not guarantee approval or inclusion in the directory.
2. Application & Review Process
All provider applications are reviewed manually by the BLKMYNDZ team.
As part of the review process, BLKMYNDZ may:
• Verify professional licenses and credentials.
• Review supporting documentation submitted by the applicant.
• Conduct credentialing and screening procedures described in our Verification & Credentialing Policy.
• Request additional documentation or clarification before making a final decision.
BLKMYNDZ reserves the right to approve, deny, or request additional information for any application at its sole discretion.
Applicants will be notified of the final decision by email.
