At BLKMYNDZ, we are committed to maintaining a trusted, professional, and culturally affirming provider network. Our Verification & Credentialing Policy explains how we review provider applications, verify professional credentials, and maintain the integrity of our directory.

Every application submitted to BLKMYNDZ undergoes a manual review before a provider profile is approved for publication.

Submission of an application does not guarantee acceptance into the BLKMYNDZ Provider Network.

Our credential verification process is designed to support the integrity of the BLKMYNDZ Provider Network and promote confidence in our directory. While we make reasonable efforts to verify information submitted by applicants, no verification process can eliminate all risk or guarantee future conduct, professional performance, treatment outcomes, or the continued validity of a provider’s credentials after approval. Users are encouraged to exercise their own judgment when selecting a provider and to communicate directly with providers regarding any questions about their qualifications or services.

BLKMYNDZ may request additional documentation or clarification before making a final determination.

Applications remain pending until the review process has been completed.

As part of our credentialing process, BLKMYNDZ may conduct certain compliance screenings where applicable, including:

• National Sex Offender Registry searches.

• Office of Inspector General (OIG) Exclusion List searches.

• Other verification procedures reasonably necessary to support the integrity of the Platform.

Completion of these screenings does not constitute an endorsement, certification, recommendation, or guarantee of a provider’s services.

5. Additional Information

Applicants may be asked to provide additional documentation, including:

• Professional licenses.

Approved providers are responsible for maintaining accurate, complete, and current information within their provider profile.

Providers must promptly notify BLKMYNDZ of material changes, including:

• License suspension or revocation.

• Changes to professional credentials.

• Disciplinary actions affecting licensure.

• Changes to practice information.

• Any circumstance that may affect eligibility for participation in the BLKMYNDZ Provider Network.

8. Reverification

BLKMYNDZ reserves the right to conduct periodic reverification of provider information to maintain the integrity of the directory.

Providers may be asked to update documentation or confirm professional information at any time.

Failure to cooperate with reverification requests may result in temporary suspension or removal from the Platform.

9. Suspension or Removal

A provider may be suspended or removed from the BLKMYNDZ Provider Network for reasons including, but not limited to:

• Submission of false or misleading information.

• Failure to maintain required credentials.

• Violation of the Provider Agreement.

• Violation of the Community Guidelines.

• Conduct inconsistent with the professional standards and mission of BLKMYNDZ.

• Failure to respond to verification requests.

10. Limitation of Verification

While BLKMYNDZ makes reasonable efforts to verify provider information, credential verification is conducted based on information available at the time of review.

Verification should not be interpreted as a guarantee of future professional conduct, competency, treatment outcomes, or ongoing licensure status.

Users are encouraged to make independent decisions regarding their healthcare providers and to contact providers directly with questions about their qualifications, experience, or services.
